Job Description:This position is onsite. Non-remotePerforms all tasks related to health information document management,

including document pickup, document preparation, scanning and importing of

documents, indexing and quality control of documents into the electronic

health record. Analyzes health information to determine clinical content and

purpose of documents for indexing into the patient medical record according to

company policy and SCL Health's designated record set. Ensure health

information documentation is available timely for patient care and healthcare

operations.Scope

Responsible for picking up medical record documents from inpatient and

outpatient areas, accurately prepping, scanning, importing or

uploading, and indexing medical records. Must possess the ability and skills

to review paper and electronic medical records to determine which documents

are appropriate for the medical record.

Ensures medical record documentation is processed into the correct patient

medical record, correct encounter, and correct document type by reviewing

each page for correct patient identifiers (MRN, HAR, CSN and patient name).

Reviews document content and assigns a document type for accurate display

of documents in the electronic record to meet the needs of physicians and

other healthcare providers.

Performs quality control to ensure the electronic document/image posts to

the correct patient's visit and location, and all pages are accounted for

with corresponding images prior to committing the batch to the EHR system.

Adheres to defined quality competencies per policy.

Audits scanned documents and electronic information to identify issues,

revise documents and correct errors, and escalate any unresolved issues to

Data Integrity or manager per procedure.

Responsible for the integrity and quality of the documents that are scanned

into the document management system, tracking of unidentified documents,

re-scanning documents, assigning document types to unknown documents.

Efficiently works with multiple, complex computer programs and hardware

components to provide quality checks for both accuracy of content assignment

and image quality.

Monitors designated queues and determines appropriate action when

necessary.

Promotes mission, vision, and values of SCL Health, and abides by

Performs other duties as assigned.

High School Diploma or Equivalent requiredPreferred:

Associate or Bachelor's Degree in healthcare related field preferred

A degree may substitute for years of experience requiredCertificationPreferred:

Registered Health Information Technologist (RHIT) or Registered Health

Information Administrator (RHIA) preferredExperienceRequired:

Six months previous office experience and/or in a healthcare setting requiredPreferred:

Experience in a HIM department setting preferredA degree may substitute for years of experience requiredTo perform this job successfully, an individual must be able to perform each

1. Knowledge of medical terminology.

skills, computer skills.

5. Ability to navigate through EMR / Content Management systems

(preferably Epic and OnBase) and locate information or identify source of

required information.
