Wherever we go we engage faith, relationship-building, and volunteers to lift-up and support Colorado’s most vulnerable citizens.SUMMARYThe Service Coordinator is directly responsible for the service provision and housing assistance for the residents in the Affordable Housing Portfolio. This program serves seniors and persons with disabilities located one of the affordable housing sites. Service modalities include Trauma Informed Care, Harm Reduction, and Motivational Interviewing. This position will work in conjunction with the Community Administrator at the housing site.ESSENTIAL DUTIES AND RESPONSIBILITIES

Responsible for implementing the Service Coordinator program in accordance with HUD and VOA guidelines.

Within program guidelines, facilitate and accept referrals on residents from the Community Administrator, family members, service providers and the residents themselves.

Assess services needed by residents in the building through surveys, visitations, group meetings, etc.

Responsible for identifying and serving the unique needs of the resident and utilizing screening and assessment.

Facilitates the delivery of appropriate services for the households as indicated by relevant service modalities through an equity lens.

Provide residents information on specific services and/or program providers, giving several options whenever possible.

Complies with all Fair Housing requirements.

Encourage the resident and/or family members to arrange for required services when possible, to maintain and encourage their independence.

Coordinate and monitor the services and activities of providers to residents to assure quality of care and cost effectiveness.

Maintain up-to-date files on service providers in the community by installing and becoming knowledgeable with the United Way database and Benefits Check-up.

Maintain and submit accurate and updated records on all residents to include the services they receive.

Develop support groups and workshops in response to needs and facilitate /co-facilitate these groups on a regular basis.

Assist in relocating residents as needed and securing emergency housing.

Support team members by serving as a resource for analyzing and solving problems.

Attend orientations, trainings, education programs, staff meetings, community meetings, conferences and workshops as requested. May provide training and mentorship to team members and community stakeholders regarding best practices in relevant service models and practices.

Clearly document all client interactions along with required eligibility and demographic information.

Perform duties in a professional manner by maintaining the confidentially of all information and by participating effectively within and across teams.

Complete annual CEU requirements.

Performs job responsibilities in accordance with the Social Work Code of Ethics.

Predictable, consistent attendance.

Perform all other duties as assigned.

RequirementsQUALIFICATIONS

Bachelor’s degree in human services, social work, or a closely related field or related experience.

At minimum, one full year of working with senior citizens and/or people with disabilities.

Willing to obtain first aid, automated external defibrillator and cardiopulmonary resuscitation certifications and keep current.

Criminal and civil background checks within 30 days of hire.

Excellent interpersonal skills and good organizational skills.

Proficient in using Windows environment and working with databases.

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS

Works in an indoor office environment with prolonged periods of sitting.

Some local travel required to fulfill training requirements and attend meetings.

Out of state travel once per year to attend national conference.
