We are currently seeking a Part-Time Health & Wellness Services Assistant. Candidates must be comfortable working with older adults in a multi-cultural setting. Reliability is a must.

SUMMARY: The Health and Wellness Services Assistant will be responsible for providing support to Kavod Senior Life’s Health & Wellness department. The primary objectives of the role are to arrange appointments, coordinate with providers, manage reminders and maintain timely and accurate records.

ESSENTIAL DUTIES AND RESPONSIBILITIES Include the following (other duties may be assigned):

■ Maintain the master calendar of provider clinics and resident visits; Arrange appointments, provide residents and other patients of the H&W Center with routine reminders of scheduled appointments, complete necessary paperwork, and provide escorts as needed.

■ Stays apprised of service provider offerings and maintains a high level of customer service with residents, providers, staff, families and community members to ensure that Kavod’s standards for quality service are achieved.

■ Supports the Health & Wellness team with data entry of HWDs, posting appointments in PCC, data entry of sign in sheets for classes, application completion (see specifics below) and submission, etc.

■ Provides H&W program support by; creating and distributing fliers, reminding residents, supporting set up and take down, ordering supplies.

■ Works with the Health & Wellness team and other staff to assist with special projects and/or requests. Attends regular staff meetings as scheduled by the director and/or Kavod’s leadership team. Completes necessary required training in a timely fashion.

■ Supports care coordinators by; helping residents gather appropriate support documentation for and completing SSDI/AND/HCBS/PTC applications (as needed).

■ Assists residents in appealing denied applications with the support of the care coordinator.

■ Maintains confidentiality in accordance with HIPAA regulations.

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE: Must be 18 years of age or older with a high school diploma or general education degree (GED) and he/she must read, write and speak English. Demonstrated success working with detailed documentation and processes is essential. Working knowledge of Microsoft Office Suite is required. Experience working with the elderly is preferred. Russian language skills are also helpful but not required.

LANGUAGE SKILLS: Ability to read, write and comprehend simple instructions, short correspondence, and memos. Ability to effectively present information in one-on-one and small group situations. Must have good communication skills and ability to work well within the organization as a team member and understand the importance of confidentiality.

MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide.

REASONING ABILITY: Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems in standardized situations.

PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. WORK ENVIRONMENT: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee is regularly exposed to general office machinery and equipment and may occasionally be exposed to risk of electrical shock. The noise level in the work environment is usually moderate.

Schedule: Monday-Friday, 30 hours/week, between 8:00am-5:00pm.
