Care Foundation Services is actively seeking energetic and professional Host Home Providers in the Longmont, Boulder, Loveland, Fort Collins, Westminster, Thornton, North Denver, South Denver, Wheatridge, Lakewood, Highlands Ranch, Colorado Springs , Arvada, Auroa and Centennial areas to offer a home to adults with intellectual and developmental disabilities. Host Home Providers offer an in-home placement to adults with disabilities of varying needs allowing each person to live in their own communities with the support they need to stay as independent as possible.

Care Foundation Services focuses on making lasting placements and will match an individual in services with you based on the compatibility of you, your family, and your home environment. No two Host Homes look alike and we are flexible in developing a residential support plan that matches your needs with the needs of a person seeking placement. We support a diverse set of people with varying needs and are looking for homes and providers to match!

Host Home Providers are contractors of Care Foundation Services and receive competitive payments for the difficulty of care required for the person. Care Foundation Services provides comprehensive general training as well as specific training to the individual in care. Our residential team is on-call 24/7 and is there to support you if any needs come up during the placement. You become a part of our integrated care team and we aim to create placements that integrate fully into the community and your home.

Care Foundation Services will provide:

General training on caregiving and the needs of individuals with disabilities

Trauma-informed and social emotional care responses

First-aid kit and general safety trainings for all members of the household

Person-centered training based on the person in care

Integrated and adaptive behavior support plans

Crisis intervention and emergency response planning

24/7 on-call support

Support team of expert clinicians to provide in-home and community-based training as needed

proactive scheduling support

Provider duties and responsibilities:

Provide personal assistance, social and emotional support, and other personal care to adults with intellectual and developmental disabilities in a home and community environment

Support with activities of daily living including shopping and meal preparation, personal care/hygiene routines, and general in-home support tasks as needed

Community activities based on the needs of the individual - can create a unique activity plan together for their respite stay

Transportation services as needed - may include transportation to/from scheduled activities and appointments

Complete documentation for services provided

Medication administration and support as needed

We have skilled providers from a variety of professional and personal backgrounds - we are ultimately looking for people who are: Passionate about actively working to improve the lives of persons with disabilities

Open to providing behavioral support as needed in an effective, calm, and caring manner. Desire to work closely within a community network of family members, individuals in services, and agencies. Willing to create a flexible, adaptive, and supportive environment to offer a home to an individual in need

Requirements for the contract:

Must be at least 18 years old

Must be able to lift at least 75 pounds

Must have a Qmap or willing to obtain one

Must have dependable and reliable transportation with the ability to safely transport

Must have a valid driver's license

Must be able to pass background check

Must have appropriate living environment with at least one available bedroom

Preferred knowledge and understanding of developmental disabilities, mental health, and trauma

Must be competent and willing to perform Non-Violent Crisis Intervention physical managements as trained

Job responsibilities include reporting any suspected maltreatment, abuse, neglect, or exploitation of an at risk individual (M/A/N/E) Each position is considered a mandatory reporter

Compensation:

Compensation varies depending on the level of support the individual needs. Angels Touch Health Care Services offers a competitive rate ranging from $0- $7000 per month.

Job Type: Full-time

Pay: $1.00 - $7,000.00 per month
