Job Description
Bear Creek Senior Living is looking for an individual to fill a temporary Social Services position on theSkilled Nursing portion of our community. Position would be for a minimum of 12 weeks with position starting late June/early July. Position would be responsible for Assessing and assisting residents in their social and psychosocial capabilities and needs, setting up and attending care conference's, assisting residents in discharge planning and arranging services upon discharge.
Qualifications
Experience and/or education in Social Work or other Human Services related educations (Psychology, Sociology, Counseling, Gerontology)
