OVERVIEW:The Douglas County Mental Health Initiative (DCMHI) Youth Care Compact
Navigator is responsible for coordinating all aspects of The Youth Care
Compact (YCC), a program of the Douglas County Mental Health Initiative
(DCMHI). As fully designed and implemented, the Youth Care Compact
(YCC) will build upon the DCMHI's Care Compact for adults, by offering
a seamless, no-fail system for children, youth and their families with
mental illness, substance use disorders, intellectual and developmental
disability/delay (IDD) and basic needs through a care coordination
network that addresses these conditions and associated needs.EDUCATION and/or EXPERIENCE:A bachelor's degree in a relevant field (i.e. human behavioral health
science, criminal justice, social work, etc.) is required. A
Master's Degree in a relevant field (i.e., Social Work, Counseling,
Psychology) is preferred.
Two years experience or more in care coordination, intensive case management
or a related field is required.
Experience in clinical or cross-system care coordination or intensive case
management is preferred.
Experience working with individuals with behavioral health needs,
collaborative groups, and community professionals preferred.
A combination of education and experience may be considered.CERTIFICATIONS, LICENSES, & REGISTRATIONS:This position requires successful completion of a criminal background check
including fingerprinting through a national database.
Must possess a valid Colorado Driver's License upon hire, with complying
insurance. Review of motor vehicle record is required at time of hire and
