For more information please visit our website at www.dignityhealth.org/woodland.ResponsibilitiesThis individual provides physical therapy services under the direction of the Physical Therapist and according to the physician's plan of care.

Provides direct physical therapy according to directions of Physical Therapist and in accordance with the physician's plan of care.

Treats the patient to relieve pain, develop or restore function and maintain maximum performance.

Directs and aids patients in active and passive therapeutic exercises, muscle re-education, gait and functional training, transfer activities and prosthetic training.

Utilizes physical agents, including, but not limited to, heat, cold, water, light, electricity, ultrasound and massage, appropriate to the home setting.

Observes and reports to the Physical Therapist and the agency personnel, the patient's reaction to treatment and any changes in the patient's condition.

Instructs patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

Maintains accurate and complete agency-required documentation and other necessary records, clinical notes and conference notes which will be incorporated into the patient's clinical record within defined time frames.

Participates in Home Health team meetings and inservice education programs.

Instructs patient's family/caregiver and other agency health care personnel in patient's treatment regime when indicated and appropriate.

Performs duties independently with a minimum of supervision but seeks guidance and direction as necessary for performance of duties.

Understands and complies with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA and other regulations and licensure requirements as they pertain to the position.

Performs other duties as assigned and/or requested.

QualificationsMinimum Requirements:Experience:

Minimum of 2 years experience as a PTA in an acute/home care/rehab setting.

Must be a licensed driver with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.

Licensure:

Current California license as a Physical Therapist Assistant.

Basic Life Support (BLS)
