Today, Dignity Health Medical Foundation works hand-in-hand with physicians and providers throughout California to provide comprehensive healthcare services to the many communities we serve.ResponsibilitiesThis position working under the direct supervision of clinic leadership exists to provide back-office and/or population health coverage support to operations when regular full-time staff in these roles are absent from work for both scheduled and unscheduled instances. The MA role provides technical and assistive patient care in support of the department to which the individual is assigned. The Population Health role is responsible for facilitating all activities related to the preventive and chronic disease management clinical practices as captured within the medical record. These endeavors involve identification and outreach verbally in writing and electronically to patients in support of aforementioned activities. Further the role works closely with the providers staff and leadership to optimize the quality of the patient care being provided.Qualifications

Minimum Qualifications:

High school diploma (or equivalent) required

Possesses a Certificate to practice as a Medical Assistant (MA Diploma or Certificate from accredited program required)

Has the ability to read, write and speak English in order to perform duties of the position
