Job Title: Independent Living Coordinator I

Supervision: Reports directly to Moving Forward ILP Services Program Supervisor and/or Executive Director of Moving Forward ILP Services.

Job Summary: Aids and provides assistance in the vocational training of disabled individuals.

Moving Forward ILP Services serves adult individuals diagnosed with developmental disabilities. We serve individuals in Tulare County, Kings County, and Fresno County. Work schedules are flexible and perfect for individuals with experience or looking for experience in the social services field.

The Independent Living Coordinator I (ILC) work with clients in a home and/or community-based environment to provide training and support in the areas of housekeeping, money management, medical appointments, educational planning, public transportation, job readiness, community resources, safety skills, self-advocacy, and socialization. The Independent Living Coordinator I is responsible for overseeing and directing tasks designed to support independent living services to consumers in our Independent Living Program with the intent of providing opportunity for success to live independently in the community. These services include the core services of Advocacy, Skills Training, Information and Referral, and Transition. The ILC I will ensure that all services are provided in accordance with the consumer’s Independent Living Plan.

Responsibilities and Duties: Under the direction of the Executive Director of Moving Forward ILP Services and Moving Forward ILP Services Program Supervisor, the ILC I will assist consumers through a process of measurable, evidence-based instruction immersed with community-based experiences which provide opportunities for consumers to practice skills, gain experience and build self-esteem and self-efficacy. The ILC I will follow an Independent Living Plan for independent living including housing, independent living and leisure skills which identify preferred options that match for that individual (i.e., physical limitations, preferences for living arrangements, transportation, preferences, etc.).

The ILC I must always maintain an attitude of professionalism and collegiality with both staff and clients.

Perform other related duties as assigned.

Education Required: High school diploma or GED certificate from an accredited national or regional agency.

Experience Required: One year of experience in a related field of industry, rehabilitation, social services, or education.

Other Special Skills & Knowledge Required:

1. Must be able to exercise sound judgment, safe work skills, initiative and organization in

directing the work clients.

2. Must be able and willing to take and pass CPR/First aid certification within 90 days of

employment.

Job Essential Elements:

3. Must be able to pass a pre-employment drug screen, physical and criminal records check.

4. Must be at least 21 years of age.

5. Must have and maintain valid California driver license and DMV record acceptable

insurance. Must maintain current vehicle insurance with a copy on file when using personal vehicle for business.

6. Must be able to individually lift 50 pounds from floor level to waist height observing appropriate

safety practice when lifting, stooping or bending and in the performance of all other job functions.

7. Must be able to push/pull/stoop/crouch/reach while performing daily job duties.

8. Must be able to use proper two person lifting techniques.

19. Must work cooperatively with co-workers, trainees, third party agencies and customers. Must respect their rights, including the right to privacy, dignity, and confidentiality.

10. Must be at designated work site when scheduled unless otherwise excused by supervisor.

11. Must demonstrate punctuality in the performance of all job duties.

12. Must be able to communicate clearly and concisely, in English, both verbally and in written form. (Spanish speaking a plus)!

13. Ability to respond effectively and have strength and agility to assist with clients who have mobility, behaviors, and/or physical limitations.

14. Observe all Moving Forward ILP Services departmental policies and procedures.

15. Must be able to multi-task.

16. Work schedule as assigned: Monday-Saturdays, 8am-8pm. holidays or irregular shifts as required to meet business need and complete contract requirements.

17. Must be able to work outside year-round in various climates to include inclement weather.

19. Must be able to complete all other duties as assigned.

Job Duties:

Document and report medical appointments and treatments.

Relate to consumers and their guardians in a warm, caring, and non-judgmental manner.

Plan and conduct community-based activities and provide ILS training in the client's own home and community.

Assist clients to apply for and manage community resources and services.

Document client activities, progress, and challenges.

Responsible for the maintenance and quality of all work contracts under their supervision.

Complete all paperwork as required, including client records.

All other duties as assigned.
