The Position:
The ECM (Enhanced Care Management) Advocate provides comprehensive assessments, care plans emphasizing whole person care, and intensive care management to populations of Focus as identified by CenCal Health. The ECM Advocate will work collaboratively with clients and public or private agencies through coordination of care. Through advocacy, planning, coordinating, and linkage to community resources, the ECM Advocate will identify risks, promote strengths, and equip clients with the skills and knowledge needed to overcome sources of stress, strengthen resiliency, and address health disparities. Services are provided in community settings, FSA offices, and/or in the home.
FSA strives to be a trauma-informed and resilience-oriented organization. Because working with those impacted by trauma can affect staff in various ways, FSA actively identifies and employs methods to support staff to embody a healthy and sustainable balance of care for both themselves and others.
Sample of Duties and Responsibilities:
1. Provide Enhanced Care Management services, most commonly face-to-face, to CenCal Health identified Populations of Focus including individuals and families who at risk of experiencing homelessness, at risk for avoidable hospital and emergency department utilization, experiencing severe mental illness or substance use disorder, children and youth involved in child welfare, pregnant and postpartum individuals who are in the birth equity population of focus, and more.
2. Provide the following seven services to participants:
- Outreach and Engagement
- Comprehensive Assessment and Care Management Planning
- Enhanced Coordination of Care
- Health Promotion
- Comprehensive Transitional Care
- Member and Family Supports
- Coordination of and Referral to Community and Social Supports
3. Complete comprehensive assessments with participants focusing on whole person care.
4. Work alongside participant in creating a comprehensive care plans that addresses physical health, mental health, disabilities, substance use, oral health, community-based long term services and supports, support to manage serious illness, trauma-informed care needs, and community and social services.
5. Provide Community Health Worker services as needed promoting overall wellbeing.
6. Exhibit a working knowledge of community resources and provide appropriate referral to support access to social, educational, and health services.
7. Timely and thorough documentation of all assessments, forms, care plans, and sessions with participants.
8. Check FSA email and voicemail every day worked.
9. Other duties as assigned.
Employment Standards:
1. Bachelor’s degree required, with preference of major course work in social work, psychology or another closely related field.
2. Bilingual (English/Spanish) required.
3. Ability to communicate ideas clearly and concisely, both verbally and in writing, is required.
4. Desired: At least (2) years of experience in social case work, eligibility determination, social service or community resource referral, or closely related work
5. Desired: Knowledge of general interviewing practices and/or techniques; knowledge of case management practices and techniques, including crisis prevention and intervention
6. Excellent interpersonal, analytical, computer, written, and verbal communication skills required.
7. Professional attitude, enjoyment of teamwork, and ability to maintain confidentiality a must.
8. Reliable transportation, valid CA driver license, and proof of insurance required to travel throughout Santa Barbara.
Classification, Hours and Pay Rate:
- This is a full-time position that offers a comprehensive benefits package, which includes medical, dental, and vision insurance, along with generous sick leave accrual, a limited number of holidays, and eligibility for flexible spending accounts and a 403(b) retirement plan.
- The pay range for this position is $26 - $27/hour, depending on the candidate's education and experience
