Community Choice Active Life Center provides day program services, positive behavior support and community integration assistance to adults with developmental disabilities.
Position: Community Integration Facilitator
Full-time: Monday-Friday 7am-3pm or 9am-3pm
Starting Pay: $20.00 per hour. Rate will increase depending on experience and education.
As a Community Integration Facilitator your duties include but are not limited to providing support and direct supervision to day program participants while participating in activities in the community and at the day program center.
We provide company vehicles to transport participants to activities and additional staff support.
Community outings include bowling, movies, library, outdoor games in the park, attending Friday Club at Cal Poly, picnics at local parks and beaches.
Our center activities include music therapy, crafts, leisure games, learning new skills, improving skills, and assisting with activities of daily living. We also offer pre-vocational activities to our participants which includes acquiring skills in hospitality and basic office support.
Qualifications:
High School diploma or GED required.
Must be 18 or older.
Have a valid California D.L
Clean driving record with two years of verifiable driving experience.
Pass background check, physical ability exam and negative TB test.
