This pivotal role as the TAY Families Coordinator is an opportunity to lead and support Transition Age Youth (TAY) heads of households grappling with homelessness. You'll play a role in providing housing focused case management, facilitating resource navigation, passionately advocating, and forging collaborations with stakeholders to ensure accessible housing solutions and comprehensive supportive services. The Ideal Candidate

We are searching for a committed and creative professional with a background in social work or related fields, showcasing exceptional communication skills, and a deep-rooted passion for tackling homelessness among Transition Age Youth. The ideal candidate will demonstrate a proven track record in case management and effective resource navigation and an unwavering commitment to applying equity principles in service delivery is crucial.

Experience working collaboratively within diverse teams is highly desirable. Moreover, individuals who have personally navigated homelessness during their early years or encountered challenges after exiting the foster care system will be considered assets. Patience and adeptness in navigating bureaucratic systems are additional qualities we highly value.

Required Qualifications

Bachelor’s degree in Social Work, Psychology, Human Services, or related field (or a combination of experience)

4 or more years of experience working with homeless populations, preferably TAY and families.

Knowledge of housing services, case management, resource navigation, and community engagement

Proficiency in data entry and management, preferably with HMIS or similar systems

Strong communication, empathy, and interpersonal skills

Major Responsibilities

In collaboration with Director of Family Programs, develop and implement TAY family programing across all Homeward Bound programs that adequately results in outcomes that meet program objectives.

Provide comprehensive case management services to TAY families, crafting individualized housing and service plans.

Act as the primary contact for TAY families in the Coordinated Entry System, guiding them towards suitable resources and housing options.

Advocate for the needs of TAY families in partnership with stakeholders and community resources.

Ensure accurate and up-to-date documentation of client interactions and progress within HMIS.

Establish and maintain open and collaborative relationships with colleagues.

Support the Program Director in their absence.

Greet, communicate, and treat all program participants with respect, dignity, kindness, and an attitude of compassionate service.

Maintain confidentiality related to all program participants.

Communicate clearly with supervisor and members of the Leadership Team.

$75K -$85K annually with a competitive benefits package which includes 100% employer paid medical and dental insurance, accrued vacation hours, and access to a 403b retirement plan. Salary and hourly rate offers are determined based on final candidate qualifications and experience.
