Summary of Position:
Under the general supervision of the Direct Services Supervisor and within the scope of the agency's mission, the Case Manager is responsible for: communicating effectively and respectfully with clients and stakeholders, exercising discretion and independent judgment, meticulous record keeping, meeting with clients and landlords in person, conducting eligibility determinations and supporting eligibility compliance efforts, conducting strengths and needs assessments, case management, budgeting, housing search supports, home visits, landlord advocacy and negotiations, developing individualized phased rental subsidies and housing retention plans, reporting and contributing to program and agency evaluation and audit activities.
Responsibilities:
Assess and screen participants to determine program eligibility in accordance with program and documentation guidelines.
Offer eligibility compliance support to referred participants
Provide housing search supports to participating families including assisting with identification of housing preferences; identification of potential rental listings and resources; preparing a master rental application, one on one landlord interview coaching; attending open houses; identifying potential project and voucher based long-term housing opportunities and projected post-exit rent limits based on anticipated income and household budget.
Assist families in identifying their needs and strengths, clarifying options and developing short and long-term goals.
Linkage to community resources which promote long-term housing retention.
Provide outreach and education on program services to community programs as needed.
Act as client advocate in securing appropriate housing, employment, benefits, education, counseling, health care, child care, transportation, treatment and other needed services.
Development of comprehensive housing service plans that promote housing retention, address individual challenges as well as promote and celebrate achievable goals.
Perform Home Visits at minimum on a monthly basis once a family is housed.
Provide landlord advocacy and negotiation on a case by case basis.
Facilitate rental agreements with landlords in scattered site rental units and ensure clients understanding of rental agreements and lease compliance.
Development of individualized temporary phased rental subsidy plans
Offer, as appropriate and applicable, Peer Counseling
Offer skill building and tools which promote successful transitional budgeting as families move from homelessness to housing stability
Offer monthly budgeting skills and tools to promote housing retention post-exit
Offer positive tenancy skill building and support positive community relationships.
Extensive record keeping, data entry and documentation of services.
Coordination of services with community based and government social service providers.
Attend case reviews, meetings, training, workshops and conferences related to position and as assigned.
Attend and participate in a minimum of 2 agency sponsored fundraising/outreach events each year, as assigned.
Review reports and summarized data and provide meaningful contribution to program and agency evaluation activities
Maintaining confidentiality at all times.
Completion and submission of reports as assigned and needed.
Submitting completed rental assistance requests accurately and promptly.
Performing data entry and client status updates by specified due dates as assigned
Maintaining good communication and work as a team with other staff and service providers.
Maintaining clear and concise records.
Follow all agency and program policies and procedures
Other duties as assigned.
