Wellness Nurse

The Wellness Nurse is the leader and responsible for monitoring Home-health, Hospice and incident report communication and follow ups then given to Health and Wellness Director for review for need for state report and final to Executive Director. The Wellness Nurse is involved in residents care, interventions and recommendations when there is a significant change of condition. The Wellness Nurse is also responsible for the monthly weights, wound weekly assessment and documentation and weekly hospice assessments with documentation. The Wellness Nurse is to assist the Health and Wellness Director with tasks and in her absence, share the responsibilities for the daily care of residents at the property with the Health and Wellness Director and Care Managers.

Reports to: Health and Wellness Director.

ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES

1. In the absence of the Health and Wellness Director, the Wellness Nurse is to Communicate with community healthcare professionals, including physicians, discharge planners, nurses and state agencies on the care needs of all residents at the community.

2. Perform nursing duties as directed and in accordance with the appropriate scope of the Nurse Practice Act, including administering medications and performing treatments as ordered by physician.

3. Conduct thorough reporting of pertinent information regarding resident care to oncoming shift. Complete documentation in and encourage use of the Communication Log on each shift.

4. In the absence of the Health and Wellness Director, Participate in end-of-shift report to ensure all needed information is communicated.

5. In the absence of the Health and Wellness Director, Assist Executive Director with physician orders and pharmacy delivery of medications. Organize central storage and delivery of medications for each shift.

6. Observe residents and collect data pertinent to resident care. Communicate all relevant resident information with physicians and other healthcare professionals as needed.

7. Ensure proper documentation regarding change in condition and incident reporting for both residents and staff. Ensure all paperwork is completed and that proper documentation is maintained in the medical record.

8. Assist Health and Wellness Director, Care Managers and Medication Aides in the performance of their job requirements and assume caregiver and/or medication aide activities when needed.

9. Respond promptly to resident and staff needs. Carry a pager and radio at all times while at the property.

10. In the absence of the Health and Wellness Director, assist both Care Managers and residents in performing activities of daily living, including; shaving, dressing, oral hygiene, toileting, bathing, incontinence care, feeding, nail care, transferring in and out of beds/wheelchairs, and the setting and clearing of tables at meal time.

11. Use Universal Precautions and follow established policies concerning exposure to blood/body fluids.

12. Along with the Health and wellness Director, work towards maintaining each resident’s self-respect, personal dignity, personal safety and confidentiality.

13. Display tact and friendliness when dealing with residents, families, visitors and co-workers.

14. In the absence of the Health and Wellness Director, assist new residents with arranging personal items and help familiarize residents and their family members with their new surroundings.

15. Attend all scheduled in-service classes.

16. As requested, assist Executive Director with orienting new staff to the residence.

17. Assist in putting chart together for new admission for Health and Wellness Director to input.

18. Provide support to community’s marketing efforts through positive/friendly interaction during all property tours and telephone contact with the public, and as directed by a supervisor.

19. Maintain a safe and secure environment for all staff, residents and guests, following established safety standards. Perform daily and nightly rounds of the property. Notify maintenance staff of all needed repairs.

20. Encourage teamwork through cooperative interactions with co-workers and other departments.

21. Support a positive and professional image through actions and dress.

22. Performs other duties consistent with the position as assigned by the Executive Director.

23. Coordinate training for caregivers to include MedTechs with the help of the RCC.

24. Covering shifts as needed and sharing responsibility with Health and Wellness Director alternating months in covering staff call off’s when RCC is unavailable and assist with any other tasks that help keep the community in compliance.

25. Along with the Health and Wellness Director, coordinate monthly Health & Wellness Clinic.
