Private Home Health Case Manager Needed (Neurological)

Private, part-time Home Health Case Manager needed to manage the care of a 79 y.o. female with a late-life movement disorder similar to Parkinson's disease (progressive supranuclear palsy). She requires assistance with managing caregivers, medical service providers, health care, health insurance, and services acquisition. Client lives in Alameda. Life expectancy is 3-7 years.

RESPONSIBILITIES

Assess and support client and family needs

- Assess the client’s needs and goals. Ensure care needs are met as per the client’s and the family’s wishes. Reframe these goals with the patient and family as the patient’s health status changes. Clearly describe the person’s healthcare goals to the patient, family, and care team.

- Assess family members’ needs and goals. Ensure that care meets family’s needs.

- Monitor the success of caregivers and medical service providers in meeting client’s and family’s needs.

- Incorporate the client’s and family’s feedback into care support and management.

Hire and manage caregivers and medical professionals

- Hire, train, coordinate and monitor caregivers and medical service providers.

- Ensure that the client’s ADLs, nutritional needs, medications, GERD management, physical therapy, and speech-language therapy needs are met.

- Ensure continuous home care

- Work to ensure positive health outcomes and the well-being of caregivers, family members, and healthcare providers.

Coordinate and manage client’s healthcare

- Address and manage medical concerns as they arise

- Work with primary care physician and specialists to rapidly resolve healthcare issues

- Manage and procure patient medications, medical equipment, and would care. Ensure adequate and appropriate supply and delivery.

- Collaborate with caregivers to guarantee travel to and from medical appointments and advocacy / representation at medical appointments

Manage insurance and assistance

- Evaluate medical insurance against patient and family needs

- Recommend and implement adjustments as needed

- Seek and find available resources to better support patient care

REQUIREMENTS

- Licensed clinical social worker or RN with a specialty in neurology

- 2-4 years of case management experience

- Proactive and personable nature
