Qualifications:
High School Diploma or equivalent. AA or BA preferred.
Computer skills to include Microsoft Word and Excel, 35 WPM.
Ability to interact effectively and in a supportive manner with persons of all backgrounds.
Sound decision-making skills including problem solving, critical thinking, and good judgment for clinical and non-clinical issues.
Excellent written and verbal communication skills; English/Spanish bilingual preferred.
Reliable transportation, a valid and current driver’s license and automobile insurance is required.
Flexible and adaptable to change.
Excellent organizational and time management skills with the ability to work independently.
Ability to learn quickly, time management and retain complex information.
Essential Duties/Responsibilities:
Advocate for patients and help them achieve their best health.
Be highly dependable and independent, as much of the work will take place in various settings, medical office, specialists, hospitals and patients’ homes.
Conduct outreach and enrollment assessments.
Create a care plan that reflects patients’ needs.
Connect patients to resources to meet these needs.
Serve as a liaison between the community and health and social services.
Collaborate well with clinic staff, community, and Care Neighborhood team.
Meet documentation standards for work assignment.
Adhere to policy and procedures of assigned clinic.
Perform other duties as assigned.

Physical Demands and Working Conditions:
Work is normally performed in a typical clinic office work environment (and, in some cases, telecommuting sites). The physical demands described here are representative of those that must be met by an employee to perform the essential functions of this job successfully. Physical: Occasionally required to carry/lift/push/pull/move up to 20lbs. Frequently required to perform moderately difficult manipulative tasks such as typing, writing, reaching over the shoulder, reaching over the head, reaching outward, sitting, walking on various surfaces, standing, and bending. Occasional travel to other Axis health centers and other occasional travel will be required.
Equipment: Frequently required to use repetitive motion of hands and feet to operate a computer keyboard, telephone, copier, and other office equipment for extended periods.
Sensory: Frequently required to read documents, written reports, and signage. Must be able to distinguish normal sounds with some background noise, as in answering the phone, interacting with staff etc. Must be able to speak clearly, understand normal communication, and be understood. Must be able to see clearly and have the ability to judge distances and spatial relationships to see objects where and as they actually are.
Cognitive: Must be able to analyze the information being received, count accurately, concentrate and focus on the given task, summarize the information being received, accurately interpret written data, synthesize information from multiple sources, write summaries as needed, interpret written or verbal instructions, and recognize social or professional behavioral cues.
Environmental Conditions: Frequent exposure to varied office (medical clinic/office) environments. Rare exposure to dust and loud noises.
