Full Time Case Manager
Mountain Vista Farm is seeking a Case Manager to work for one of the longest standing residential substance use disorder treatment programs in Northern California.
We are looking for a counselor to join our team of talented clinicians and provide individuals struggling with substance use disorders an opportunity to have a healthier more productive life.
Mountain Vista Farm has been providing detox and addiction treatment since 1949 for adults. This 8-acre facility is in Glen Ellen, CA and is one of the few privately owned treatment programs in Northern California.
Job Description
Facilitate individual, group and family counseling sessions
Prepare individual treatment plans, tailored to meet the clients’ goals
Evaluate clients’ goals and facilitate ongoing services to support their recovery
Complete and maintain accurate clinical documentation on each client
Review and evaluate clients progress according to their needs
Attend trainings to increase clinical knowledge and awareness

Education/Requirements
Registered or Certified Alcohol and Drug Counselor with a minimum of 1 year experience. CPR (training is available upon hire)

Pay: $25.00 - $29.00 per hour
