ESSENTIAL DUTIES/RESPONSIBILITIES:

Under the direction of the RN Manager:

The Care Transition Registered Nurse (aka CTRN) will be responsible for providing care transitions services for patients who have had inpatient care or an emergency department visit to ensure that they are being discharged to settings appropriate for their needs and that they are connected to a medical home and a primary care provider. Responsibilities to include but not limited to:

Identification of patients in need of care transition services using EHR notification pools and external data sources such as claims or MCO dashboards

Evaluation of patient hospital record including obtaining necessary records not available in the EHR health information exchange

Coordination with hospital and health plan management before and after discharge

Provides ongoing communication with patients post-discharge regarding care by telephone and also through secure messaging, secure Internet, or other asynchronous non-face-to-face consultation methods (for example, email or secure electronic patient portal)

Assessment of patients for acute care needs or medical complications

Medication Reconciliation

Provision of care coordination services for complex patients as needed, including home health, DME, transportation, financial assistance, and linkages with community resources

Initiation of appropriate in house referrals

Provision of timely follow up appointments

Participation in regularly scheduled strategy meetings

Implementation of quality improvement recommendations including enrollment of patients into chronic disease care programs to improve quality of care for high-risk members

Requires strong clinical care skills & experience, including systematic assessment, planning, evaluation, monitoring, patient/family education and documentation in accordance with all program requirements

Perform other duties as assigned

REQUIRED COMPETENCIES:

Demonstrate clinical assessment skills at the level necessary to meet the job requirements

Ability to make independent decisions when circumstances warrant such action

Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to medical care

Effective interpersonal skills and the ability to work with an interdisciplinary care team

Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, and policies and procedures that are necessary for providing quality care

Capable of researching new methods and principles to incorporate them into existing nursing practices

Proven decision making and analytical skills

Ability to organize, prioritize, and efficiently carry out duties and assignments

Ability to interact effectively and in a professional, supportive manner with persons of all backgrounds

Excellent verbal and written communication skills

Computer skills, to include Word and the ability to use the Axis practice management systems and Electronic Health Records

QUALIFICATIONS:

Current active California Registered Nurse license

Minimum of 1 year experience preferably in case management or acute care setting

Experience and knowledge of primary care medical services

Ability to meet requirements of CPR, First Aid and other mandated programs

Physical Demands and Working Conditions:

Work is normally performed in a typical clinic work environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential duties of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position.

Physical: Occasionally required to carry/lift/push/pull/move up to 30lbs. Frequently required to perform moderately difficult manipulative tasks such as typing, writing, etc. Must be able to stand, sit for extended periods. Requires the ability to reach with hands and arms, and occasionally climb stairs. Requires sitting operating a computer keyboard, telephone and other equipment for extended periods of time. Occasional travel to other Axis health centers and other travel will be required.

Sensory: Frequently required to read documents, written reports, and signage. Must be able to distinguish normal sounds with some background noise, as in answering the phone, interacting with staff, etc.

Environmental Conditions: Frequent exposure to varied office (medical clinic/office) environments. Occasional exposure to dust and loud noises.

Equipment: Frequently required to use a computer, phone, copy machine, fax machine, and other medical office equipment.
