We are a primary care and palliative medicine physician practice seeking a dedicated and compassionate Geriatric and Dementia Care Manager to join our team. This role is vital in providing specialized support to our patients with cognitive, neurologic, and other geriatric disorders. The Geriatric and Dementia Care Manager will work closely with our primary care providers (PCPs), neurologist, patients, and their families to enhance care delivery, improve patient and family experiences, and facilitate efficient operation within the practice.

Key Responsibilities:

Patient and Family Support: Conduct follow-up communications with patients and their families after doctor visits to discuss key aspects of the care plan and ensure proper implementation. Regularly check in with patients and families to monitor the patient’s condition and address any changing needs.

Care Coordination: Schedule and track regular medical appointments, including biannual cognitive evaluations, Annual Wellness Visits (AWVs), and advance care planning sessions. Participate in cognitive evaluation visits, aiding in the completion of assessments such as the MOCA and caregiver evaluations.

Education and Counseling: Assist in developing and distributing educational materials for patients dealing with cognitive disorders. Provide advice and counseling to families facing challenges related to these conditions. Develop and participate in practice-offered educational and support activities.

Resource Navigation: Develop a comprehensive understanding of local resources available to support patients and families. Coordinate referrals, assist with application processes, and facilitate access to these resources.

Liaison Role: Act as a key connector between patients, families, and the broader healthcare team, including external providers and organizations involved in the patient’s care.

Continuous Learning and Development: Stay informed about the latest advancements in dementia care, geriatrics, and relevant community resources to enhance patient care.

Documentation and Administrative Support: Thoroughly document all activities to improve care continuity and support chronic care management processes. Assist with clerical duties related to billing procedures for cognitive evaluations and chronic care management.

Qualifications:

Degree or significant experience in Social Work, Gerontology, Nursing, or a related field. Master’s degree preferred.

Experience in geriatric care, dementia care, or a similar role within a healthcare setting.

Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
