Job Description:
The Enhanced Care Management (ECM) Program Manager will oversee the ECM program's development, implementation, and operations.
In accordance with CalAIM, the program manager will build the capacity and infrastructure of the ECM program.
Will oversee ECM staff to provide efficient, comprehensive, and effective ECM care management services to older adults and disabled members of managed health care plans.
Qualifications:
Bachelor’s degree in social work, nursing, gerontology, health administration, human services, or related field is required
Two years of supervisorial experience
Minimum 3 years’ experience working with older adults and previous experience working with an ethnically and economically diverse population
Close knowledge of and relationship with local community
Ability to work independently with strong level of organizational skills
Fluency in English (bilingual in Spanish strongly preferred)
Previous care management experience strongly preferred
Patient Care Coordination and Healthcare experience strongly preferred
Must pass a criminal background check
Travel is required within Santa Barbara County
Must have reliable transportation, current driver’s license, clean driving record, and proof of auto insurance
Primary Responsibilities:
Build the capacity and infrastructure of the ECM program
Oversees ECM Program implementation and design processes, including data system development
Manages effectiveness of service delivery through ongoing supervision of all ECM team members
Ongoing client care coordination documentation monitoring includes assessments, care plans, community support referrals, outcome measures, and progress notes
Ensures timely and high-quality documentation of services by ECM in accordance with all agency and contractual requirements
Train, monitor, and support ECM staff to provide appropriate ECM services to Managed Care Plan (MCP) members and ensure ongoing achievement of quality measures
Ensure culturally appropriate and timely in-person ECM activities
Maintains ongoing communication with managed care providers (MCP) and other contractual partners
Ongoing communication and collaboration with MCP, provide updates on program efforts
Ensure timely outreach to eligible members
Ensure accurate data input in compliance with MCP standards and requirements
Responsible for timely ECM reporting, billing, and effectiveness of data management systems

Supervisorial Responsibilities:
The Program Manager will oversee the interviewing, hiring, onboarding, and training of ECM team members
Work with the organization as necessary to carry out supervisorial responsibilities
Provide weekly supervision to all ECM team members
