Description Job Title: Case Manager for a Non-Profit Organization Location: On-site, San Francisco (SF)Description: Robert Half is urgently seeking a dedicated Case Manager for one of our esteemed non-profit clients. The client organization is passionately driving its mission to provide support and advocacy for marginalized communities within San Francisco. This position is strictly on-site in San Francisco.Responsibilities:
Conduct comprehensive assessments, develop care plans, and manage service implementation.
Coordinate services with multiple service providers.
Attend regular team meetings and case review meetings.
Collaborate with other staff members to provide the highest quality of case management services.
Document client progress and maintain necessary paperwork in a timely manner.
Monitor the effectiveness and outcomes of the case plan.

Bachelor's degree in social work, psychology or related field required. Master’s degree preferred.
At least 2 years of experience in case management within the non-profit sector is desirable.
Solid understanding of social service resources in the SF area.
Excellent communication, interpersonal and organizational skills.
Ability to work independently, manage time effectively, and meet deadlines.
Proficiency in MS Office.
Multilingual skills are a plus.
