VCC is an equal opportunity employer.ResponsibilitiesResponsible for developing, implementing, monitoring and evaluating the Enhanced Care Management (ECM) program. Provide organizational support, program assessment, compliance and performance improvement processes consistent with the mission and goals of the organization.Provide direct supervision of project staff including personnel administration, work assignments, recruitment and hiring, reviews, disciplinary action and training activities.Prepare training curriculum and provide on-going training for project staff, clinic staff and program participants as requiredIn coordination with Clinic public relations staff, develop press releases and promotional material as neededParticipate in contract review, negotiations and amendmentsServe as a liaison and assist with collaborative support activities with community agencies, local government representatives, neighborhood residents and other Vista Community Clinic project staffResponsible for monitoring quality measures associated with ECM, developing strategies for improvements within the program and associated care teamsQualificationsMinimumBachelor’s degree in public health or related fieldMinimum three years' related experienceValid California driver’s license, current vehicle insurance and reliable transportationPreferredMaster’s Degree in public health or related fieldBilingual English/SpanishRequired Skills/Knowledge/AbilitiesKnowledge of available local community resources and government, community health centers, public assistance programs and state/federal regulationsAbility to work flexible hours, including evenings and weekends as requiredProficient in MS Office programs, spreadsheet, desktop publishing programs and business e-mail communication systemsSalary Range$70,000 - $80,000 DOE PI249959305 Apply
