VCC is an equal opportunity employer.ResponsibilitiesEnroll mentees into the program and conduct initial status assessmentsMonitor and track mentee's progress on a monthly basis and share with Lead MentorWork with mentees and families to assist them in meeting their needsIdentify assetsand experiences outside of the mentee's neighborhood to broaden mentee's world view and sense of possibility, and accompany mentee to these placesProvide intensive case management assessment and care coordination to clientsGenerate referrals to VCC and other agencies for services/resources and monitor follow-throughProvide intensive support, advice, and guidance during twice weekly group sessions, outside the sessions, and as otherwise specified in the mentee's personal case planWork with HHSA personnel to access services applicable to mentees and their familiesParticipate in weekly staff meetings with program teamLead interactive journaling, participate in other group/social activities with mentees, and provide one-on-one mentorshipQualificationsMinimumHigh school graduate or equivalent with five years' case management experience.Valid California driver’s license, current vehicle insurance and reliable transportationAvailability and willingness to work evening and weekend hours, as required.Should be community member with relevant life experience and social capital that gives authority to challenge the norms and change the thinking of those youth with whom they mentor.Have current or prior criminal justice involvement with arecord of success and be positively engaged in changeMust be at least 18 years of ageUndergo background checks conducted by Probation to comply with Board Policy C-28-Criminal Background Check History and be subject to Probation approvalPreferredAssociates degree plus four years' case management experience or Bachelor's degree in related field with two years' case management experienceKnowledge of local social service landscapeHave connections with mentee (e.g. similar neighborhood, school, shared family or friends)Bilingual English/SpanishRequired Skills/Knowledge/AbilitiesAbility to present information effectively verbally and in writing to individuals and groups from various cultural, ethnic and educational backgroundsAbility to work independently and under time constraintsAbility to handle confidential materials and information in a professional mannerPay Range$21.00PI273310873 Apply
