The Life Coach supports individual(s) with intellectual and developmental disabilities. The Life Coach supports individuals in life areas, such as work, other programming, money managements, healthy and wellness, personal safety, behaviors, and activities of daily living both in on-site and in the community.
RESPONSIBILITIES
Teaches individuals life skills such as mobility training, work and community skills.
Providing information to supervisors by telephone, in written form, e-mails, text messaging, or in person.
Participate, engage, review, and follow in individual’s daily routine(s)/activities as written in his/her Personalized Service Plan (PSP).
Ensure that the Day Program complies with the standards of operation.
Document all reports and date regarding the individual on time; this includes Daily Activity Log, Task, etc.
Assisting individual with grooming and self-care and always instruct individual in proper behavior.
Perform other related duties and assignment as required.
