OVERVIEW: The Community Mental Health Worker is responsible for providing strength and community based rehabilitative services, as described in the case plan, for the clients who have been accepted into the Counseling Center program.

DUTIES:

Work with the team to develop and implement an appropriate individual treatment plan for each client in order to meet his/her mental health needs.

Assist clients and families to engage in appropriate problem solving.

Provide individual and/or group rehabilitation services for clients such as behavioral management support, social skill building, household maintenance and cleaning, budgeting, securing safe and adequate housing, banking, personal hygiene, clothing and grocery shopping, cooking, laundry, health care, transportation, interpersonal communication, and time management.

Provide a positive role model and mentor-type relationship with clients.

Provide and teach self-advocacy and independence for families.

Participate in child and family team meetings as assigned.

Utilize community resources to help clients meet the goals of their treatment plan, referring clients to appropriate community resources as needed.

Help clients to maintain a supportive network within their own neighborhood and the greater community.

Assist and advocate for clients in working with other agencies and professionals to ensure continuity of care.

Maintain and model appropriate boundaries.

Provide crisis intervention as needed and participate in the 24/7 on-call system.

Provide transportation to appointments, meetings, and classes as needed. Log mileage and report maintenance and/or services needs as necessary.

Participate in new employee orientation tasks as assigned.

When CMHW Supervisor is absent, coordinate drive schedules as assigned.

Write and maintain accurate client case notes in Avatar as required.

Document delivery of services and complete all relevant forms in a timely and efficient manner and in accordance with agency and program policies and systems.

Provide client information to team for ongoing assessment and evaluation purposes.

Complete Child Health Questionnaire section of Initial and Annual Core Assessments as required and in a timely manner.

Participate in staff meetings, supervision sessions, training, case reviews, curriculum and other meetings as required.

Participate in case reviews as required.

Collaborate with Clinical Staff in developing the client plan and in providing services.

Participate in Sacramento Children’s Home activities and training as required.

LOCATION: 2750 Sutterville Rd. Sacramento, CA 95820 (Counseling Center)

SCHEDULE: Monday – Friday, 8:30 am – 5:00 pm

SALARY: $21.76 - $24.48 per hour, to start

QUALIFICATIONS:

At least one year of full-time experience working in the mental health field is required. 

Two (2) years of college or an AA degree in Human Services, Psychology, Sociology, Social Work, Counseling, or a related field is preferred.

Driving is an essential function of this job. As such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is also required. Must be at least 21 years old.

Must be able to read, write, speak, and understand the English language.

Knowledge of mental health issues.

Knowledge of child development.

Knowledge of cultural issues.

Knowledge of child abuse issues.

Knowledge of substance abuse issues.

Knowledge of behavior management.

Knowledge of family dynamics.

Ability to provide crisis intervention.

Ability to co-facilitate groups.

Knowledge of and ability to provide basic counseling techniques.

Ability to communicate clearly both orally and in writing.

Bilingual written and verbal skills, culturally appropriate outreach and translation skills may be required per contract or program.

Good organizational skills.

Skill to maintain a professional, confidential work environment.

Ability to manage multiple tasks at one time in an efficient manner.

Strong problem-solving skills.

Knowledge of Microsoft Word.

Ability to effectively utilize electronic health record systems such as Avatar Clinical Work Station.
