We are a fast-growing Home Health Agency based in Sacramento seeking to recruit an Intake
Coordinator to join our team. You will be responsible for patient care coordination which includes
interacting with referral sources, patients, and skilled/professional patient care team.,
Responsibilities
-Answer phones and receive all referral and intake information
-Complete intake forms and appropriately route them for admission approval
-Assist in onboarding new referral and coordinate with the care team
-Handle matters related to staff and patients they serve with exception to issues requiring
professional/clinical judgement
-Data entry into the EMR, scheduling appropriate skilled person to fill the case,
-Coordinate at between patients, care team and the Doctors/doctor offices
-Communicate promptly with referral sources to maintain smooth relations and obtain needed
information
-Coordination of onward/timely transmission of patient documentation
-Data Entry, chart creation, reporting and other healthcare related duties
-Calendar and strong scheduling on a daily basis
-Work one weekend a month on referrals and charting
-Prepare for the patient visit, including pulling charts and notes, as well as setting up medical
equipment
Qualifications
-2-3 years’ experience in Intake coordination/scheduling
-EMR/Computer skills with strong ability to navigate various systems
-Excellent customer service skills
-Strong attention to detail
-Excellent organizational skills
-Excellent Communication Skills
Work Hours – Mon-Friday (8hours) and some weekends on call when necessary.
