Amani Exceptional Services provides supported living services (SLS) to adults with intellectual and developmental disabilities. Job Description

To summarize the role of the Community Support Facilitator, the following items listed below are the key duties and responsibilities:

· Case management duties and able to coordinate the implementation of Client IPP objectives and meeting their overall support needs.

· Responsible for the overall support and services pertaining to assigned clients.

· Must possess innovative skills that will foster client independence and skills development.

· Manage, supervise and train all direct support staff working at the houses under the assigned caseload.

· Must be able to work flexible hours to meet the needs of the assigned clients; can perform the role of the direct support staff when unavailable or if no one can support the client.

· Has good verbal and written skills; able to meet deadlines with monthly progress report and other written documentation such as SIR, ISP, etc.

· Able to conduct House Team meetings and participate in all annual IPPs and Planning Team meetings.

· Willing to be On Call at least one weekend a month; twice if the need arises.

· Able to identify any issues affecting the agency or the services being provided to clients and report to management immediately.

· Other duties may be assigned by Program or Executive Director.
