Prioritizes and coordinates the plan across the care of continuum through critical thinking, teamwork, and communication between care providers, patients, families and external vendors to ensure timely discharge.Job Description :EDUCATIONEquivalent experience will be accepted in lieu of the required degree or diploma.

Associate's: AA in Business/Health related field or equivalent education/experience

PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:

1 year in a healthcare or hospital setting working directly with interdisciplinary team/patients/families

SKILLS AND KNOWLEDGE

Oral and written communication skills.

Interpersonal and time management skills

Ability to work effectively in a fast pace environment with rapidly shifting priorities and competing demands.

Ability to work independently with a minimum of direction. Ability to exercise discretion and prioritize tasks, seeking input as indicated.

Intermediate PC skills and word processing skills required.
