Join us to provide health care services in a Federally Qualified Health Center (FQHC) located in California’s beautiful western sierras. Shingletown is a small town located along California State Route 44 in the hills just below Mount Lassen. We are also known as "The Gateway to Lassen". Work as an integral part of our provider staff by participating in our medical/behavioral health team, QA/QI and additional staff teams with some home visits. You will provide and manage direct patient care, infants through the geriatric population. Our team will assist you in assuring the adequacy and appropriateness of medical care provided to patients.
We provide Whole Person Care with a support staff including RNs, MAs, case managers, telehealth nutritionist/diabetic educator, telemedicine endocrinologist, telemedicine rheumatologist, telemedicine psychiatrist, LCSW(both in-house and via telehealth), community health worker, referral coordinator, a chiropractor and other business office staff. SMC owns and operates a local fitness center, and the emergency food & outreach center. We provide video visits and home visits as appropriate.
Work Schedule:
4-10 hour days if full-time, less if only want part-time
Total of 32 hours patient care – 8 hours of admin time (full-time). Prorated if part-time.
Required Qualifications (Knowledge, Skills, Abilities, Education, Training, and Licensure):
Current and valid licensure to practice as a Nurse Practitioner or Physician Assistant in the State of California.
Current and valid California License.
Current BLS/ACLS certification.
Current DEA certificate, including Schedule II.
Complete patient charting and billing within 48 hours of patient visit.
Able to communicate effectively in English, both verbally and in writing.
Excellent interpersonal skills.
Basic computer knowledge with electronic health records experience preferred.
The FNP/PA will have appropriate backup by a physician and will not be required to provide services for which the FNP/PA is not trained.
Knowledge of current therapies and ability to involve the supervising physician when appropriate.
Previous outpatient clinic experience.
DOT physical certificate preferred.
