- SOC Start of Care
- Evaluation
- Follow Up
- Discharge / Re-certification
Qualifications
- Active (unrestricted) nursing license in the State
- 1 year in the past 3 years (professional)nursing experience in Home Health or Acute Care
- Valid Drivers's License w/ Clean DMV
