Description We are offering a long-term contract employment opportunity for a Clinical Document Improvement Specialist l in Oakland, California. This role plays a crucial role in the healthcare industry by ensuring the accuracy and quality of patient records. The specialist will be responsible for reviewing and improving clinical documentation, collaborating with healthcare professionals, and educating them about the importance of complete and accurate documentation. Responsibilities: Collaborate with healthcare teams to improve the documentation within the medical records that accurately reflect the patient's severity of illness and risk of mortality. Facilitate the Clinical Documentation Improvement work group and assist with special projects as needed. Ensure continuity of patient care by validating clinical documentation with the plan of care and making necessary referrals. Conduct initial and extended-stay concurrent reviews on selected admissions to clarify documentation for accurate reflection of severity of illness. Work closely with coding staff to review clinical issues and ensure appropriate DRG assignment. Use in-depth knowledge of DRG payer issues, documentation opportunities, clinical documentation requirements and referral policies and procedures to improve record accuracy. Use strong communication skills to clarify documentation in records with physicians, case managers, nurses, or other healthcare professionals. Conduct follow-ups on unanswered queries during the patient's stay to obtain responses to open queries. Ensure the accuracy and completeness of clinical information used for measuring and reporting physician and hospital outcomes.If you are interested, please call us ASAP at (510)470-7450Requirements Proficiency in Documentation and Reporting, with a focus on maintaining accuracy and compliance with company policies and procedures. Demonstrated ability in Planning and executing tasks effectively, ensuring quality and timely delivery. Knowledge of Human Resources (HR) Administration and Medical Coverage policies, with the ability to navigate through complex scenarios. Experience in performing Ad Hoc Financial assessments, aiding in financial planning and decision making. Familiarity with facility operations and progress tracking, able to contribute towards continuous improvement. Excellent Communication Skills, both verbal and written, to effectively liaise with various stakeholders. Understanding of Clinical Trial Operations and Coding, with the ability to handle queries and issues efficiently. Experience with Medical Records management, including the handling of patient referrals and ensuring data privacy. Knowledge of HealthCare.gov and its functionalities, with the ability to guide patients through the process. Familiarity with various research Methodologies, with the ability to Diagnose Problems and develop effective solutions. Experience in creating and executing Validation Plans to ensure data accuracy and reliability. Proficiency in data Capture techniques and managing relationships with Payers to ensure smooth transactions.
