Visit dignityhealth.org/northridgehospital for more information.Program Coordinator - Community & School Health InitiativeNorthridge Hospital Medical CenterNorthridge Hospital Medical Center is seeking a passionate and dedicated Program Coordinator to join our Community Health Work & Sustainable Outreach and Navigation (CHW SON) team. This position is grant-funded and will play a vital role in the daily supervison of Community Patient Navigators (CPN's) , relays updates and orversees the implemtaion and refinement of the CHW SON project.Responsibilities

Develop and implement program activities, including community health education workshops, outreach events, and resource referrals.

Manage program budget, track expenses, and ensure compliance with grant requirements.

Develop and maintain program materials, including curriculum, presentations, and promotional materials.

Coordinate with community partners and stakeholders to ensure program effectiveness and reach.

Identify and engage with community members, particularly those from underserved populations.

Build relationships with community leaders, organizations, and institutions.

Conduct needs assessments and community health surveys to inform program development.

Train, and supervise Community Patient Navigators (CPN's) to deliver program services.

Provide ongoing support and mentorship to (CPNs), ensuring their skills and knowledge are up-to-date.

Collect and analyze program data to track progress, measure impact, and inform program improvement.

Prepare regular reports for funders and stakeholders, highlighting program achievements and challenges.

Qualifications

BS/BA degree required in Health Administration Public Health or Public Policy degree preferred.

Master's degree preferred.

2 years of experience as a Community Health Worker or System Navigator or 5 years’ of experience as in community based programs or social services.

Minimum 1-2 years’ experience implementing community-based programs developing outreach strategies collaborating with community-based organizations and report writing.

Basic knowledge of budgeting or accounting.

Knowledge of local community agencies and resources.

Ability to organize projects and establish priorities.

Creative and analytical skills.

Ability to travel locally for outreach events (some evenings and weekends).

Grant writing experience preferred.

Presentation and public speaking skills.

Working knowledge of Microsoft 365 and Google Workspace products Zoom and Canva.

Bilingual (Spanish/English) preferred.

Fire Card (must obtain within 90 days of hire date. We offer the classes in the hospital)
