As a Nurse Care Manager (NCM) with our Multipurpose Senior Services Program (MSSP), you will play a crucial role in supporting low-income elderly participants that are at risk of moving to a licensed facility. We strive to assist them living safely in their homes and communities. Your primary responsibilities will include conducting comprehensive home health assessments, developing individualized care plans addressing healthcare needs, emergency services, nutrition, and more. You will coordinate necessary services and resources for participants, monitor and adjust care plans based on their progress, and manage medications to ensure adherence. Your role will also involve active collaboration with an interdisciplinary team, maintaining accurate electronic case records, and advocating for participants with healthcare providers. This position requires traveling within Santa Cruz and Monterey Counties to engage with diverse communities, demonstrating a commitment to inclusivity and cultural sensitivity.

To qualify for this role, you must have a valid RN license in California, a minimum of two years of RN experience with at least one year in community nursing, the ability to travel within our service area, and a commitment to working with diverse communities.
