This is a full time position with generous benefits. The organizations LVNs are responsible for providing direct patient care under the supervision of a registered nurse. This position also assists the RN Case Mangers by triaging current patients and communicating with physicians, pharmacies, and other members of the interdisciplinary team to provide quality patient care. The LVN follows an established plan of care, provides treatments, and works collaboratively with team members to achieve positive patient care outcomes.
Qualifications:
Ability to proficiently use computer systems for electronic charting;
Current California LVN license;
Valid California drivers license;
Reliable transportation for daily home visits;
Successful completion of drug screen, physical examination and criminal background check at the time of hiring.
