As a Community Health Worker Specialist, you play a vital role in supporting the Department of Health Services by conducting initial community health assessments, promoting safety net services, supporting community preparedness, and reducing chronic disease incidences. Some responsibilities include:
Providing culturally informed health education and information to diverse communities
Assisting individuals in navigating the health system to promote equitable access to healthcare
Offering health coaching and social support to community members
Implementing individual and community assessments to identify needs
Conducting outreach through home visits and presentations at local agencies and community events
Advocating for the needs and perspectives of local communities
