Job description

The case manager position provides full case management for clients seeking Information Referrals and peer support through Conversion Transition services with our ADRC Partners.

Essential Duties and Responsibilities:

Provide clients with peer counseling, and advocacy/general advocacy assistance.

Assist in facilitating housing, and benefits resources

Provide personal assistance, referrals, and individual peer counseling.

Evaluate client's needs, making appropriate referrals, including follow-up evaluating client progress

Assist clients to relocate and transition from a controlled environment and adapt to becoming more independent.

Maintain and submit accurate records for statistical information on services provided.

Effectively maintain and document case files according to ILCSC policy and confidentiality guidelines.

Participate in and attend required training and meetings.

Attend external meetings in representing the ILCSC as assigned by the supervisor.

Uses personal transportation in the performance of duties.

Perform other duties as designated by the supervisor.

Qualifications:

Knowledge of community resources.

Experience working with persons with disabilities and seniors.

Good oral, written, communication skills, organization skills, and multitasking skills.

Ability to meet deadlines and work effectively under pressure.

Willingness to help others.

Creative and flexible with good analytical and problem-solving skills.

Strong proficiency with MS Excel and other MS Office software (Word, Outlook).

Good time management skills.

Proactive and the ability to think strategically.

Ability to take initiative in completing tasks and solving problems.

Ability to develop skills and knowledge on the Americans with Disabilities Act (ADA).

A valid driver’s license or ID.

Special Knowledge/Preferred Skills:

Bi-lingual Spanish and American Sign Language (ASL).
