Starting Salary

The starting pay is $21.00 per hour. To qualify for a higher pay rate, the candidate will have to provide documentation of relevant experience and qualifications.

Job Overview

As a Community Navigator, you'll play a pivotal role in empowering individuals with developmental and/or physical disabilities. This position involves working closely with participants, their support teams, and service providers to develop Person-Centered goals and objectives. You'll facilitate meaningful community engagements, ensuring each participant’s safety and personal growth.

Key Responsibilities

Conduct assessments to determine participants' interests and needs.

Coordinate with a diverse team to support participants' integration into the community.

Advocate for participants, ensuring they have access to necessary services and support.

Develop, implement, and monitor community-based programs that align with participants' Person-Centered Plans.

Serve as a liaison between participants, their families, and community resources.

Lead and support staff in delivering high-quality, person-centered services.

Qualifications

Deep understanding of learning methods and services for adults with disabilities.

English and Spanish bilingual.

Strong case management skills and experience in community program development.

Exceptional leadership, team building, and interpersonal skills.

Proficiency in personal computer use and relevant software applications.

Ability to work independently, make well-informed decisions, and manage multiple projects.

Bachelor's degree or equivalent in a related field preferred.

Working Environment

Work Schedule

Full-Time Position (Monday to Friday, 8:00am - 4:00pm)
