Service Coordinator: Advocate Training Coordinator

Job Description Reports to ADRC Manager Job Location: Downey/Pasadena/Arcadia, CAFLSA Status: Non-Exempt

Salary: $24.00 Hour - All Benefits

Under the general supervision of the Manager of the ADRC

The Advocate Training Coordinator will follow the Independent Living philosophy to provide various independent living services to persons with disabilities while coordinating various opportunities for consumers by utilizing the Advocate Training Specialists for success. These Services include the core services of Advocacy, Systems Change, Skills Training, Peer Counseling, Information and Referral. Additional assistance is provided by directly and/or indirectly providing resources and training for locating and obtaining housing, benefits, transportation, personal care attendants, medical services, employment or education, nursing facility transition, youth transition as well as other individualized need areas.

Essential Job Duties and Responsibilities

1. Complete intake on consumers, assess their needs, and develop an Independent Living Plan. Establish goals and time guidelines to ensure effective and efficient delivery of services. Monitor the progress of consumers toward stated goals.

2. Coordinate Services closely with Advocate Training Specialists at SCRS (Housing, Benefits, Assistive Technology, etc.) to ensure a high-quality and comprehensive IL Plan is developed to meet the consumer's needs.

3. Develop a comprehensive independent living skills training curriculum that includes a variety of areas leading to the development of a plan of support and/or training.

4. Provide a copy of the IL Plan to the consumer, and provide follow-up opportunities to assess ongoing needs, progress toward goals, and needed resources.

5. Advocate for and assist consumers in obtaining benefits such as Social Security Disability Insurance, Supplemental Security Income, Medi-Cal, Medicare, Food Stamps, and other county, state, and federal benefits.

6. Work with the Housing Specialist to locate appropriate, affordable, and accessible housing for the consumer.

7. Advocate for consumers concerning all independent living services, and provide information and referral, as well as follow-up on an ongoing basis.

8. Provide peer support and peer counseling.

9. Develop and maintain good working relationships with community service leaders.

10. Be familiar with the legal requirements of local, state, and federal programs, and other related providers to be able to provide assistance, guidance, and advocacy to both consumers and family members.

11. Assist with various application processes as needed. This may be done directly, seeking assistance from Specialist staff as needed.

12. Track all referrals and or leads provided to consumers for follow-up.

13. Coordinate training and assistance on topics identified in the Individualized Plan.

14. Compile and report performance statistics monthly.

15. Handle and organize a high volume of paperwork.

16. Represent the organization to educate the broader community about people with disabilities and available services through the organization.

17. Work with bilingual consumers, as needed.

18. Assist and work with the Consumer and the Advocate in the preparation of consumer’s cases for Appeal Hearings.

19. Maintain consumer files in accordance with SCRS Policies and Procedures, Data Base, and grant compliance standards.

20. Work with other ILC staff members to ensure that consumers receive all services needed.

21. Refer consumers to additional programs that are related to Peer Support and System Advocacy.

22. Work with community agencies to provide consumers with additional needed services, which SCRS does not provide.

23. Participate in a variety of training opportunities to continuously improve knowledge and skills as it relates to independent Living.

24. Represent the organization at outreach and other events. 25. CalFresh Contracting duties include providing presentations, Pre-screening, application assistance, and follow-up for current ILC consumers.

26. All other duties as assigned.

Qualifications

Preferred Qualifications

Bachelor's in Education, Human Services, Organizational Leadership, or related degree and/or three to five years experience in an agency that provides services to seniors or people with disabilities; thorough understanding and embracement of Independent Living Philosophy and disability culture. Understanding of legal requirements of other related organizations (Local, State, Federal, Public or Private). Must have the ability to build relationships with other service providers and know where to go to find assistance.

Work collaboratively with management and staff positions

Excellent writing and communication skills

Exercise time management skills and can multi-task.

Problem-solving, and conflict-resolution skills

Knowledge of basic Social Security, Housing, Transportation, Medical and other benefits

Knowledge of Olmstead and transitional services

Experience working with diverse individuals of multiple cultures and disabilities.

Bi-lingual preferred but not required

Public speaking required

Knowledge Administration and Management - Knowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resources. English Language - Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar. Psychology - Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; and psychological research methods. Education and Training - Knowledge of principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and the measurement of training effects. Sociology and Anthropology - Knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, cultures, and their history and origins. Clerical - Knowledge of administrative and clerical procedures and systems such as word processing, managing files and records, stenography and transcription, designing forms, and other office procedures and terminology. Public Safety and Security - Knowledge of relevant equipment, policies, procedures, and strategies to promote effective local, state, or national security operations for the protection of people, data, and property. Customer and Personal Service - Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.

Law and Government: Knowledge of laws and government regulations, executive orders, agency rules, and the political process.

This position may also place this individual on boards, committees, civic organizations, and other action groups in which this person may, at the direction of the CEO, engage in limited discretionary actions and/or decisions that will reflect impact and/or influence the business environment and image of SCRS.

This person faces decision-making, which involves critical thinking and analysis. There is a frequent need for intuitive, on-the-spot, problem resolution. Decisions that have the potential to impact SCRS's operational or strategic operations need to be referred to the CEO or designee for discussion and resolution.
