All Star Hospice is seeking a compassionate and dedicated Spanish speaking Registered Nurse (RN) in the LA area to join our hospice team. The ideal candidate will have experience in hospice care and be capable of performing admissions, supervisory visits, and recertification visits for patients receiving hospice care at home. SPANISH speaking is a must.
Responsibilities:
-Conduct admissions assessments for new hospice patients, including comprehensive evaluations of medical, psychosocial, and spiritual needs.
-Perform supervisory visits to assess patient care needs, provide support and education to caregivers, and ensure compliance with hospice care plans.
-Conduct recertification visits to evaluate patients' ongoing eligibility for hospice care and update care plans as needed.
-Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and other healthcare professionals, to provide holistic care to patients and families.
-Provide compassionate and skilled nursing care, including symptom management, pain control, and end-of-life care.
-Maintain accurate and timely documentation of patient assessments, care plans, and interventions in accordance with regulatory requirements and agency policies.
-Requirements:
-Current licensure as a Registered Nurse (RN) in the state of CA
-Strong assessment, communication, and interpersonal skills.
-Ability to work independently and collaboratively within a multidisciplinary team.
-Compassionate and empathetic approach to patient care.
-Valid driver's license and reliable transportation required.
