Responsibilities include:
Assign proper diagnosis coding in accordance with coding standards and compliance.
Complete and review OASIS for appropriateness.
Ensure timely OASIS completion for exporting to CMS.
Transmit OASIS every Month.
Demonstrate proficiency in OASIS completion
Responsibility for reviewing admission and on-going patient documentation including
OASIS to ensure that it is completed correctly and that the Plan of Care is reflective of
and appropriate to the assessment.
Resolve exporting and clinical documentation issues.
Maintain confidentiality of all Agency and patient related information. Protect patient
rights as defined in the Patient Bill of Rights.
Good knowledge of Healthcare Synergy EMR
Ensure that the POC is being followed in the discharge of patient care.
Reconcile patient medication to ensure that it is up to date and matches what is in the patient home.
Identify CMS procedural changes as needed.
Keep up with industry standards and attend annual coding/OASIS training.
