Job Summary:
We are seeking compassionate and skilled Licensed Vocational Nurses (LVNs) to provide high-quality nursing care to patients in their homes. The Home Health Field LVN will work under the supervision of a Registered Nurse to deliver patient-centered care, including administering medications, monitoring health conditions, and assisting with daily living activities. This role requires excellent clinical skills, strong communication, and the ability to work independently in a dynamic, community-based environment.
Key Responsibilities:
Provide direct patient care in the home setting, including administering medications, wound care, catheter care, and other prescribed treatments.
Monitor and document patient vital signs, symptoms, and changes in condition, reporting findings to the supervising RN or physician.
Assist patients with activities of daily living (ADLs), such as bathing, dressing, and mobility, as needed.
Educate patients and their families on health conditions, treatment plans, and self-care techniques.
Follow individualized care plans developed by the RN or physician, ensuring compliance with all medical orders and protocols.
Maintain accurate and timely documentation of patient care, including progress notes and updates to care plans.
Collaborate with the healthcare team, including RNs, physicians, therapists, and home health aides, to ensure coordinated and comprehensive care.
Adhere to infection control practices, safety standards, and agency policies.
Respond to patient and family inquiries with empathy and professionalism.
Travel to patients’ homes within the assigned service area, managing time effectively to meet scheduling needs.
Qualifications:
Current and valid Licensed Vocational Nurse (LVN) license in California.
Minimum of [1-2 years] of nursing experience, preferably in home health, skilled nursing, or a related field.
Valid driver’s license, reliable transportation, and auto insurance (required for field visits).
Current CPR certification.
Strong clinical skills, including medication administration, wound care, and patient assessment.
Excellent communication and interpersonal skills, with the ability to build rapport with patients and families.
Ability to work independently and manage time efficiently in a field-based setting.
Proficiency with electronic medical records (EMR) and basic computer skills.
Physical ability to perform nursing tasks, including lifting and assisting patients as needed.
Preferred Qualifications:
Experience in home health or hospice care.
Bilingual skills (e.g., Spanish, [other languages relevant to the area]).
Knowledge of Medicare/Medicaid regulations and home health documentation standards.
Work Environment:
