Job Title: In-Office Home Health LVN

Position Summary:
The In-Office Home Health LVN provides administrative and clinical support within a home health agency setting. This role focuses on coordinating patient care, supporting field staff, assisting with documentation, and ensuring compliance with healthcare regulations. Unlike traditional LVN roles in the field, this position is based in the office and centers around case coordination, communication, and clinical documentation.
Key Responsibilities:
-Support RN Case Managers and other clinical staff with coordination of patient care plans.
-Review and audit clinical documentation for accuracy, completeness, and compliance with agency standards and Medicare guidelines.
-Communicate with physicians, field staff, patients, and caregivers to ensure timely updates and care coordination.
-Enter and maintain accurate patient data in the electronic medical record (EMR) system.
-Track patient visits, ensure timely submission of required paperwork, and follow up on outstanding documentation.
-Prepare and organize case conference materials.
-Assist in intake process by gathering necessary clinical and demographic information.
-Perform quality assurance (QA) checks on clinical documentation.
-Educate and support field staff on documentation requirements and policies.
-Participate in team meetings and contribute to performance improvement initiatives.
Qualifications:
-Current and valid LVN license in California.
-Minimum of 1 year LVN experience in home health.
-Familiarity with Medicare/Medicaid guidelines for home health.
-Strong knowledge of medical terminology and clinical processes.
-Proficient in EMR systems (Synergy)
-Excellent organizational and communication skills.
-Ability to multitask in a fast-paced office environment.
-Detail-oriented and committed to accuracy and compliance.
Compensation: $35-$40 per depending on experience. Full benefits after 90 days of employment.
