If you are looking for Part-time/Full-time or just want to pick up few patients, we have a position available for you. We are seeking applications from experienced, hard working, knowledgeable and dependable RN candidates to join our team.
Responsibilities:
Provide skilled nursing services at the patients’ residence based on POC frequency and instructions.
Reconcile medication
Monitor and document reactions and patient progress using observation, assessment, and evaluation skills.
Educate patients and family members according to disease process, medications, treatment plans, and best practices
Report to physician and Case Manager of adverse findings.
Follow infection control policy in and out of the office.
Document visits in EMR system (DSL) in a timely manner.
Requirements
1 year experience as a home health RN field nurse ( Required)
RN License to practice in California (Required)
CPR Certification (Required)
Driver's License (Required)
Fluent in English (Bilingual is a plus)
Proof of auto insurance
Proof of Malpractice Insurance
