Field CHHAs:
Looking for motivated CHHA’s to enable patients to stay in their homes by monitoring and recording patient condition such as taking and recording vitals; Temperature, Pulse, Respiration, Blood Pressure. Providing support and personal services; Bathing, Light house-keeping, Assistance with ADL's, Assistance with transfers and teaching families.
Field LVNs/LPNs:
Looking for companionate, skilled LVN's/LPN’s to perform nursing duties within the LVN/LPN scope of practice and under physician's orders. Assess, treat and educate patients/care givers. LVN's/LPN’s will be working directly with Patients and office Case Managers. Must be organized, have excellent people's skills as well as communication skills. Responsible for timely and accurate completion of client notes, reports, and other documentation related to the treatment and progress of clients.
Field RN's:
Looking for well skilled, passionate Field Registered nurses to provide care at home. Must have home health experience and be familiar with Medicare/JACHO guidelines. RN's will be responsible for following a physician established plan of care under the direction of the Director. Must have Exceptional Assessment and Documentation skills Must be experienced in IV infusions, wound care, G-Tube, TPN, PICC lines, Ostomy Care , etc. All candidates must be reliable and paperwork must be turned in on a timely manner.
Candidate with experience in wound cares, injections, Foley care, etc. is a big plus
Field MSW’s:
Looking for a part time or full-time Medical Social Worker to work for Home Health Agency in the Los Angeles, County. Social Work is a rewarding career for self-motivated, organized, and compassionate individuals who work well with minimal direction, as well as having excellent interpersonal and analytical skills.
