Responsibilities:

1. Personal Care Assistance: Help clients with activities of daily living (ADLs) such as bathing, grooming, dressing,

and toileting, while respecting their privacy and dignity.

2. Medication Management: Assist clients with medication reminders and ensure medications are taken as prescribed,

following appropriate protocols and safety guidelines.

3. Mobility Support: Aid with mobility, transfers, and ambulation, utilizing proper techniques and mobility aids as

necessary.

4. Meal Preparation and Feeding: Prepare nutritious meals based on dietary requirements, assist with feeding when

needed, and encourage proper hydration and nutrition.

5. Emotional Support: Offer companionship, engage in meaningful conversations, and provide emotional support to

clients and their families.

6. Cognitive Stimulation: Engage clients in cognitive activities, mental exercises, and memory enhancement exercises

to promote mental acuity and well-being.

7. Safety Monitoring: Observe and report any changes in clients' physical or mental condition, promptly addressing

any concerns or emergencies.

8. Record Keeping: Maintain accurate and up-to-date records of client care, including daily activities, observations,

and any notable changes or incidents.

9. Transportation: Provide transportation for clients to medical appointments, therapy sessions, or other necessary

outings.

10. Collaboration: Communicate effectively with the care team, including nurses, therapists, and other healthcare

professionals, to ensure coordinated care delivery.

11. Client Advocacy: Advocate for clients' needs, preferences, and rights, respecting their autonomy and promoting their

self-determination.

12. Adherence to Policies and Regulations: Follow agency policies, procedures, and relevant regulations to ensure the

highest standards of care and safety are maintained.

Qualifications:

High school diploma or equivalent (additional healthcare certifications or training are a plus).

Prior experience working in a similar role or providing care for individuals with neurological disorders or chronic

illnesses.

Knowledge of common neurological disorders (such as Alzheimer's, Parkinson's, multiple sclerosis) and chronic

illnesses (such as diabetes, heart disease).

Familiarity with relevant caregiving techniques, including personal care, medication management, and mobility

assistance.

Compassionate and empathetic nature, with the ability to build rapport and establish trusting relationships with

clients and their families.

Strong communication skills to effectively interact with clients, their families, and the care team.

Excellent observation skills and the ability to respond appropriately to changes in clients' condition or behavior.

Reliable, responsible, and committed to maintaining confidentiality and privacy of clients.

Valid driver's license and reliable transportation (if required for client transportation).

Ability to work independently, follow instructions, and adapt to changing client needs.

Physical stamina and the ability to lift, bend, and perform physical tasks required for client care.
