Job Description:

We are seeking caring and dedicated Certified Home Health Aides (CHHAs) and Licensed Vocational Nurses (LVNs) to join our hospice care team. In these roles, you will provide essential support and care to patients in their homes, helping to improve their quality of life and providing much-needed assistance to their families.

Key Responsibilities:

For CHHAs:

Assist patients with personal care needs, including bathing, dressing, grooming, and toileting.

Provide support with mobility and transferring, ensuring patient safety at all times.

Assist with light housekeeping, meal preparation, and feeding as needed.

Monitor and record vital signs and report any changes in the patient's condition to the RN Case Manager.

Provide emotional support and companionship to patients and their families.

Maintain accurate and timely documentation of care provided.

Participate in team meetings and ongoing training sessions.

For LVNs:

Provide direct patient care according to the plan of care, including administering medications, wound care, and other nursing procedures.

Conduct regular patient assessments and communicate findings to the RN Case Manager and other members of the care team.

Educate patients and their families about their condition, medications, and the hospice process.

Coordinate with other healthcare professionals to ensure seamless care delivery.

Document all patient care activities accurately and in a timely manner.

Participate in on-call rotations as needed.

Qualifications:

For CHHAs:

Valid CHHA certification in the state of California.

Minimum of 1 year of experience in hospice, home health, or related field.

Strong interpersonal and communication skills.

Compassionate, patient, and empathetic nature.

Ability to work independently and as part of a team.

Reliable transportation and a valid driver's license.

For LVNs:

Valid LVN license in the state of California.

Minimum of 1-2 years of experience in hospice, home health, or related field.

Strong clinical skills and knowledge of palliative care principles.

Excellent communication and interpersonal skills.

Ability to work independently and manage time effectively.

Reliable transportation and a valid driver's license.
