Adult Day Health Care center seeks a CA licensed LVN who can demonstrate coordinating and assessment skills, along with ability to be flexible and adaptable to a changing environment. Also demonstrate skills and experience to assess and monitor elderly health care; maintain accurate written and oral reports; use of sound judgment in emergency situations; deal effectively with families, staff and community. Ability to assume other roles as needed in the course of program activities.

OVERALL OBJECTIVES:

Perform and document specific nursing tasks and procedures within LVN Scope of Practice under the supervision of a Registered Nurse in an integrated setting of Adult Day Care (ADP), & ADHC (CBAS) programs on a daytime basis.

MEDICAL ADMINISTRATION

The LVN will be responsible for the administration of P.O. medications for patients which includes monitoring, administering and recording prescribed medications.

OBTAIN & DOCUMENT VITAL SIGNS

Respiratory rate

Heart rate

Blood Pressure

Oxygen Saturation

Weight

Blood Pressure

Temperature

Blood Glucose levels as needed

MEDICAL OCCURANCES

The LVN will be available to help the RN as directed by the RN in dealing with a variety of medical occurrences, incidents, falls, etc within the LVN Scope of Practice.

NURSING FLOW SHEETS

The LVN is responsible to maintain the Daily Nursing Intervention Log for each patient, coordinated by the RN, includes both LVN and RN interventions.

PATIENT CLINICAL SERVICES

The LVN is responsible to assist the RN with nursing tasks as directed by the RN within the LVN Scope of Practice.

MINIMUM QUALIFICATIONS:

Licensed L.V.N. in state of CA; NEW GRADS WELCOME; demonstrated experience in coordinating, assessment skills, supervision and training.

Demonstrate skills and experience to assess and monitor elderly health care; maintain accurate written and oral reports; use of sound judgment in emergency situations; deal effectively with physicians, families, staff, and community.

CONDITIONS OF EMPLOYMENT:

Must be screened for and free of tuberculosis and pass a physical health screening.

Must have a current CA driver’s license / CA ID.

Must provide a criminal background clearance.

Must maintain and keep current: First Aid, and CPR training.

Pay: $20.00 per hour
