JOB DESCRIPTION

TITLE: SOCIAL SERVICES COORDINATOR – Del Norte County

HOURS: Full-time 35 hours/week (non-exempt, hourly position)

GENERAL DESCRIPTION

Under direct supervision of the Social Services Manager, this position will increase awareness and access to A1AA social services in Del Norte County. This position is hybrid with hours worked from home and scheduled days at the Del Norte Senior Center in Crescent City. This position may be asked to work weekends to support outreach events. The Social Services Coordinator works closely with the A1AA Social Services team located in Humboldt County and shares office space and client referrals with the Health Insurance Counseling & Advocacy Program (HICAP) staff in Del Norte.

A1AA Social Services Program in Del Norte County includes providing resources and referrals to older adults and adults with disabilities; volunteer recruitment and oversight, as well as outreach at community events and to sites that serve eligible older adults;

Senior Planet digital literacy classes providing technology instruction to older adults in small-group settings to reduce social isolation and strengthen connections to community supports;

CalFresh support includes application assistance to eligible 60+ recipients over the phone and in-person, and CalFresh Healthy Living (SNAP-Ed) programs supporting healthy, active, and nourished lifestyles. Programs include evidence-based classes such as Bingocize and Food Smarts.

SPECIFIC DUTIES

1. Acquire and maintain a thorough and current knowledge of older adult and caregiver support services, benefits, programs, and activities specifically within Del Norte County.

2. Respond to client telephone, walk-in, and written requests for assistance.

3. Complete relevant assessments to determine the client’s needs and interests for long-term services and supports.

4. Conduct outreach targeting older adults, especially low income, and minority populations, by tabling at community events and providing information to other organizations serving the same population. Document results of outreach

activities and attendance;

5. Teach CalFresh Healthy Living approved education sessions. Topics include but are not limited to, nutrition, food safety, physical activity, and food resource management (i.e., Food Smarts, Bingocize).

6. Recruit, train and supervise volunteers offering CalFresh Healthy Living and/or Senior Planet digital literacy classes.

7. Manage inventory of technology devices, ensure distribution of devices to eligible clients, maintain digital literacy program calendar, and complete class data entry.

8. Submit website updates to ensure accurate program information and visibility.

9. Maintain a relationship with partner and referral agencies, including but not limited to Adult Protective Services (APS), Tri-County Independent Living Center, Home Health, Medical and Emergency Response partners. Ensure other agencies have the most up to date program information.

10. Data entry and documentation of all interactions with clients, contractors, and other service providers, including but not limited to telephone calls, emails, letters, walk-in contacts, appointments, and home visits into the appropriate program software in a timely manner.

11. Become an expert on all aspects of CalFresh application assistance, helping older adults apply and acquire the necessary documents.

12. Maintain knowledge of A1AA and partner agencies accurate resource information by updating information to capture changes in community services.

13. Attend A1AA agency, program, and community meetings and trainings, as requested.

14. Maintain positive and regular communication with the Social Services Manager and report any challenges and concerns related to work duties immediately.

KNOWLEDGE AND ABILITIES REQUIRED

1. Ability to communicate with older adults, adults with disabilities, their caregivers, and their families while demonstrating respect, sensitivity, and support.

2. Strong interpersonal and presentation skills. Ability to effectively instruct in a small or large group setting.

3. Strong customer service skills including the ability to say no and hold professional boundaries. Professional and polite interactions with co-workers, clients, service providers, and the public;

4. Excellent planning, prioritizing, and organizational skills, attention to detail, and ability to meet deadlines;

5. Ability to work independently, adhere to Agency policies and guidelines, and consistently exercise good judgement to analyze and solve problems;

6. Ability to function as a cooperative, fun, and flexible team member and relate to people of diverse backgrounds;

7. Computer skills, including Google and/or Microsoft Office. Experience with various databases preferred;

8. 18-years of age or older. Valid driver’s license, insurance, a clean driving record;

9. Ability and means to travel independently and extensively around Del Norte County (including rural areas) and occasionally to Humboldt County.

In the event of a community or agency disaster, job duties and responsibilities may change as directed by the Executive Director or other authorized agency representatives. These duties would be related to assisting our client population, senior service providers, the agency, or other community organizations in disaster and recovery operations following the agency disaster plan.
