Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high-quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education, and more.Overview:Family Support Services comprises five key programs: AFLP, TCM, Cal Learn, Alameda County Public Health Home Visiting, and Cal-SAFE programs. These programs primarily serve Southern Alameda County, which includes Hayward and the Tri-Cities area (Union City, Newark, and Fremont).Targeted Case Management (TCM): TCM is a comprehensive case management service designed to assist eligible individuals under the Medi-Cal program, particularly those within specific target groups, in accessing essential medical, social, educational, and other necessary services. TCM encompasses various service components, including needs assessment, goal setting, individual service planning, scheduling services, crisis assistance planning, and ongoing evaluation of service effectiveness. This approach ensures that the evolving needs of Medi-Cal-eligible individuals are continually addressed, offering a wide range of options to meet those needs.Cal Learn Program: Cal Learn is a mandatory program that delivers the same AFLP case management services mentioned earlier. The Alameda Social Services Agency refers eligible teens to this program, who must be under the age of 19, pregnant and/or parenting, lack a high school diploma or GED, and receive Cal WORKS (cash aid). Cal Learn goes further by providing transportation, childcare, and necessary school supplies to support these young parents in their educational journey.Alameda County Public Health Home Visiting Program (ACPH): The ACPH Home and clinic visiting program offers comprehensive case management support to pregnant and parenting families with young children up to age 5. This program focuses on promoting the health, development, and well-being of young children, preparing them for school. It employs a multi-disciplinary approach, providing more intensive family-centered services in various settings. Teens and young adults are identified and referred to this program by Hospital Outreach Coordinators (HOC) from local hospitals, TVHC providers, clinic staff, and community partners.The primary responsibilities of the Family Support Services Case Manager include active participation in multi-disciplinary care teams within TVHC, aimed at delivering integrated healthcare services to clients. This role involves facilitating educational and support groups, conducting health screenings and psychosocial assessments, engaging in child development activities, providing parenting education, and coordinating individual service plans. These services are carried out in clients' homes, TVHC clinics, and within the community.This is a full-time position working 40 hours per week, typically Monday through Friday with periodic Saturday hours.Compensation: $30.34–$32.19 per hour, depending on experience. TVHC offers compensation ranges that are determined by a thorough market-based analysis and are fully disclosed in accordance with California law. The pay for a selected candidate is determined by a variety of factors to ensure fair and equitable compensation. These factors include the candidate's experience, education, skills, training, licensure, certifications, and the specific scope of the role. Provide case management services to up to 28 families each month and adhere to visit standards defined in contract scopes of work.

Develop individual service plans with each client to meet the health, psychosocial, educational, vocational and financial needs of pregnant and parenting teens, adults and their families.

Monitor client progress to determine effectiveness of service delivery and to assess progress towards individual goals and objectives.

Conduct psychosocial assessments with pregnant and parenting clients.

Engage In individual and group supervision by presenting client cases to assess needs and develop interventions.

Provides supportive counseling as needed and assess and make all appropriate internal and community referrals.

Engage clients in twice a month sessions at minimum.

Conduct home visits with clients to maintain communication and assess the adolescent and their children’s living environment.

Maintain client information and case documentation in a confidential and professional manner.

Link clients to the school system for enrollment into high school or continuation programs, junior high school or post-secondary education/vocational programs.

Assess the entitlement of childcare, transportation, and ancillary services for Cal Learn clients.

Provide clients with knowledge about pregnancy, child development, parenting, nutrition, family planning, and educational options.

Participate in bi-annual peer chart audits for quality assurance monitoring.

Participate in maintaining a network of services and resources for clients in Southern Alameda County.

Assist with outreach services to identify and bring in eligible clients into the program.

Participate in internal and community Trainings.

Perform other duties as may be required.

Requirements

Bachelor’s degree in social work, psychology, or human services field, or

Associates degree with two years of case management experience in a health and human services field or

An individual who has four years of experience performing case management duties in a health and human services field.

Previous experience working with adolescent population.

Bilingual in English/Spanish required.

Desire to work with an ethnically diverse adolescent population.

Excellent written and verbal communication skills, demonstrated by ability to communicate effectively and relate congenially to students, families, school employees, providers, and demonstrated by clearly written documentation, service plans, reports, memos, etc.

Able to meet case documentation requirements regarding content and timelessness.

Understands the importance of, and able to preserve client confidentiality.

Nonjudgmental and highly ethical.

Knowledge and understanding of health information related to parenting and pregnancy.

Able to work effectively independently and as a team member with co-workers and other agency staff.

Cultural sensitivity and sensitivity to issues specific to families with young children.

Ability to work on interdisciplinary basis with school system and health teams.

Able to meet or exceed the Service Excellence Standards of TVHC, Inc.

Have the ability to travel to various site locations as directed by the program.
