OverviewDignity Health @ Mercy Hospital of Folsom is the community hospital serving the Sierra foothills communities of Folsom El Dorado Hills Granite Bay Cameron Park Shingle Springs and Rescue. Mercy Folsom received Among America’s 250 Best Hospitals recognition and is a Primary Stroke Center.ResponsibilitiesResponsibilitiesThe incumbent provides medical nutrition therapy to individual patients through assessment of nutrition status, recommendation for nutrition interventions and education of patients. Acts as a liaison between patients and Nutrition Services. Acts as a resource for nutritional expertise to physicians and other health care professionals. Coordinates the clinical nutrition program: works in collaboration with the Food & Nutrition Services Manager to hire, fire and counsel direct reports. Facilitate orientation and training of clinical nutrition staff; develops and revises clinical policies and procedures; coordinates and facilitates clinical nutrition staff meetings; develops and or reviews patient instructional materials, develops, monitors and reports clinical nutrition QI/PI activities. Monitors competency of clinical nutrition staff. Provides nutritional education to community members via outpatient programs and community events. Demonstrates competency in patient care for neonate, pediatric, adult, and geriatric patients. Clinical Nutrition staff includes Dietitians, Diet Technician and Diet Assistants.

Demonstrates knowledge to provide appropriate care to all age groups served. Exceed: No complaints and has documented expertise in all age groups (neonatal, pediatric, adolescent, adult, geriatric) Meets Expectations: Experience and expertise in 4-5 of the above age groups. Does not Meet: Needs additional training in age-related medical nutrition therapy and/or counseling as evidenced by 3 or more validated complaints or observations of failure to provide adequate education or assessment related to age-specific needs.

Performs and documents nutritional assessments according to facility standards; estimates nutritional needs appropriately based on age, gender, stress factors, diagnosis, weight, cultural issues, intake status, etc and evaluates patient tolerance to therapeutic diets when appropriate. Exceed: greater than 95% of charts reveal nutrition needs are assessed according to facility standards. Meet: 85-95% of charts reveal nutrition needs are assessed according to facility standards. Does not Meet: Less than 85% of charts reveal nutrition needs are assessed according to facility standards.

Educates patients, family members and caregivers. Education method is appropriate for age, cultural and psycho-social needs and living conditions of patient. Exceed: greater than 95% of charts reveal patient education needs are assessed and provided appropriately and education is documented on the Interdisciplinary Teaching Record (ITR). Meet: 85-95% of charts reveal patient education needs are assessed and provided appropriately and education is documented on the ITR. Does not Meet: Less than 85% of charts reveal patient education needs are assessed and provided appropriately or education is not consistently documented on the ITR.

Demonstrates concise, competent medical record documentation. Exceed: greater than 90% compliance to department and facility documentation standards (including ITR and POC-I). Meet: 80-90% compliance to department documentation standards. Does not Meet: less than 80% compliance to department documentation standards.

Collaborates with other hospital services to plan and implement patient care as necessary in meeting the nutritional needs of the patients. Proactively communicating patient's nutritional and issues. Exceed: Attends greater than 90% of interdisciplinary rounds and discusses nutritional issues with physicians in order to facilitate implementation of nutritional recommendations. Meet: Attends rounds 80-90% of the time. Does not Meet: Attends less than 80% of the time or feedback from the interdisciplinary team that employee needs to improve in giving nutritional input during rounds.

Provides timely patient follow-up and monitoring of nutrition plans; refers to interdisciplinary plan of care, when doing follow-ups. Exceed: N/A. Meet: Complies with follow up standards greater than 90% of the time. Does not Meet: Complies with follow up standards less than 90% of the time.

Demonstrates knowledge and application of Process Improvement (PI). Exceed: Documented evidence of improved quality or improved systems as a result of QI/PI projects and completes PI monitoring and written report on time 90% of the time. Meet: Completes assigned PI monitoring and written report (on time) 90% of the time. Does not Meet: Completes assigned PI monitoring and written report less than 90% of the time.

Particicpates in the hiring, orienting and training of nutrition staff. Reviews the competencies of the RD's, DT's and cooks in regards to clinical skills and food production. Exceeds: Actively trains, orients and evaluates competency of nutrition staff Meets Expectations: Helps orient and train nutrition staff. Does not Meet: Does not help train nutrition staff.

Facilitates the orienting and training of clinical nutrition staff. Exceeds: Actively recruits and trains clinical nutrition staff. Meets: Helps recruit and train clinical nutrition staff. Does not Meet: Does not help recruit or train clinical nutrition staff.

Approves patient menus and nutritional supplements.

Maintains pertinent patient data necessary to recommend, prescribe, or modify therapeutic diets as needed to meet the nutritional needs of the patients.

QualificationsMinimum Qualifications

2 years experience as a Clinical Dietitian in an acute care setting with progressive clinical & foodservice responsibilities

CA Registered Dietitian in good standing with the Commission on Dietetic Registration and American Dietetic Association.

American Dietetic Association registration and membership. Dietitians hired as registration-eligible must become registered within 3 months of hire. Certified Nutrition Support Dietitian (certified by the American Society for Enteral and Parenteral Nutrition).

Bachelor's degree in Nutrition, Dietetics or related field.
